Payment & Insurance

Currently Functional Medicine Associates does not accept Medicare, Medicaid or third-party insurance. We are a direct pay cash-based practice with payment due at time of service. We do offer payment plans for those individuals who need the cost of visits divided into several payments.
While we do not take insurance directly, Functional Medicine Associates accepts HSA/FSA payments as an out-of-network licensed medical provider. In addition, we will also provide a SuperBill with the correct ICD-10 and CPT codes for the patient, so they may submit for reimbursement from their private insurance carrier. Many of the private insurance carriers have streamlined this process online.
Due to federal regulations, Superbills cannot be submitted to Medicare or Medicaid for reimbursement. This is not a policy of our practice but a restriction set by Medicare and Medicaid regulations.
Superbill’s are becoming a standard model for many functional and integrative medical providers, therapists, PT/OT and out of network providers who could not offer their services otherwise. Policy may vary between insurance providers, but typically a good portion of the medical visit fee can be reimbursed (depending on the individual’s deductible). Most of our visits can be coded as a 99205 for new patient visits, or a 99214 for a 30-minute follow-up.Â
